A CARF Three-Year Accreditation was awarded to St. Coletta’s of Illinois, Inc. for the following programs/services:


Community Employment Services:  Employment Supports

Community Employment Services:  Job Development

This program is designed to provide community-based employment opportunities to persons with severe disabilities that require intense support to maintain employment in a community setting.  An array of services is provided to ensure each individual placement.  The services provided could include:  assessment of the individual; the development of an employment profile; the matching of the individual to a job; developing an employment opportunity for the individual; providing time-limited intensive job coaching; counseling the employer and employees to assist in the individual’s successful transition and the fading of the job supports.  Each program participant has an Annual Employment Plan developed by program staff upon entering the program. The plan is reviewed and adjusted monthly or as needed.




  1. Must be eighteen (18) years of age or older.

  2. Must have a documented disability that prevents competitive employment in an integrated community setting.

  3. Must have a valid social security number.

  4. Must have a valid state ID or driver’s license.

  5. Must be certified free of communicable diseases by an M.D.

  6. Must have funding in place at the time of admission                   (see Referral Process).

  7. Must display an interest in program participation.

  8. Must be able to obtain transportation or transport self to and from program and work sites.

  9. Must not possess a criminal background or behaviors that will interfere with the individual obtaining or maintaining employment.
